"These results add clarity to the genetic architecture that is playing a role in arsenic toxicity and its underlying biology," said senior author Habibul Ahsan, MD, MMedSc, Louis Block Professor of health studies, medicine and human genetics at the University of Chicago Medicine. "It's a rare type of study for a major problem affecting millions of people around the world, and it opens up opportunities for genetic studies of other major public health problems in developing countries."

The group's genome-wide association study, or GWAS, was conducted in nearly 3,000 individuals exposed to arsenic for decades in Bangladesh. Since the widespread installation of hand-pumped wells to tap groundwater sources in the 1970s, as many as 77 million people ? about half the population of Bangladesh ? have been accidentally exposed to dangerous levels of arsenic. The World Health Organization calls the exposure "the largest mass poisoning of a population in history."

For more than a decade, Ahsan and colleagues have studied the epidemiology of arsenic-related disease, such as skin lesions, diabetes, and respiratory illnesses, in this population, as well as the effectiveness of interventions to prevent toxicity. In the new study, funded by the National Institute of Environmental Health Sciences and the National Cancer Institute, the researchers sought genetic answers for why some individuals appear to be at higher risk for developing disease after arsenic exposure.

"Whatever the source of exposure, different individuals vary with respect to their susceptibility to the toxicity of arsenic," Ahsan said. "Even if they consume or are exposed to arsenic at the same dose and duration, some individuals will manifest toxicity phenotypes and others won't."

Researchers genotyped thousands of arsenic-exposed individuals from the group's main studies for single nucleotide polymorphisms (SNPs) throughout the genome, and looked for associations with arsenic metabolite levels and risk of skin lesions.

MMA is considered to be more toxic, while DMA is water-soluble and more easily excreted. Higher levels of DMA or lower levels of MMA measured from an individual's urine are associated with lower toxicity.

A research team led by Ahsan and Brandon Pierce, PhD, an assistant professor of epidemiology at the University of Chicago Medicine, then used GWAS to search for SNPs associated with DMA and MMA levels. They found several significant genetic variants in the region of a likely candidate gene: arsenite methyltransferase (As3MT), an enzyme critical for arsenic metabolism. A second GWAS looking for SNPs associated with the development of skin lesions after arsenic exposure, pointed to many of the same variants. In a further study of gene expression levels, those same SNPs were associated with reduced expression of the arsenic metabolizing enzyme.

"This makes perfect sense," Ahsan said. "It gives us a very coherent story that we can now investigate in relation to other arsenic pathologies and in relation to a wide range of arsenic doses in this population."

"Now that we understand the molecular basis of some of this disease risk, it is conceivable to now think of incorporating this information into testing, evaluating, or potentially coming up with successful biomedical interventions," Ahsan continued. "By exploiting these metabolic pathways for a subgroup of individuals who will really be at higher risk for getting those diseases, we may be able to reduce fatal outcomes in this population."

The genetic findings provide strong evidence that efficient metabolism of arsenic through methylation protects against the toxin. Compounds that boost methylation, such as folic acid, could reduce arsenic toxicity ? a strategy currently being tested by co-author Mary Gamble, PhD, associate professor of Environmental Health Sciences at Columbia University.

"If we could somehow find a way to do that in Bangladesh, it would make individuals much better methylators of arsenic, and as this current study shows if you're a better methylator you're at a lower risk for disease," said co-author Joseph Graziano, PhD, Professor of Environmental Health Sciences and Director of Superfund Research Program at the Mailman School of Public Health of Columbia University.

Risk variants may also help assess the potential toxicity of cancer chemotherapies which use arsenic or related compounds. SNPs associated with elevated sensitivity to arsenic toxicity could steer oncologists toward lower doses or alternative treatments in certain cancer patients, Ahsan said.

Beyond the clinical applications, the current study demonstrates that large-scale genomic studies are possible in a largely rural population of a developing country. The study offers a rare example of a GWAS result with clear, immediate potential for translational impact.

"Many genomic signals that we see are not robust enough or do not pertain to a large population," Ahsan said. "But in this study, that is not the case. The finding is robust, and the impact is massive."

Chris Weigant: Friday Talking Points -- My "War on Women" Rant

"When the going gets weird," Hunter S. Thompson famously said, "the weird turn pro."

This quote has been running through my head all week. It's been such a weird week, in fact, that I even wrote a column praising a Mitt Romney campaign ad. Weirdness abounds, in other words. Well, I was really just wonkily praising the cars in the ad, but still...

Speaking of weirdness, we had what could be the final Republican presidential debate. In it, the three top Republican candidates explained how they were going to -- simultaneously, mind you -- both bring down the cost of gasoline and bomb Iran. This qualifies not just as weird but downright bizarre. I mean, how stupid do they think the American public is? Well, I suppose it's just the Republican primary electorate, but even so...

When the going gets weird, the weird run for president, apparently.

The most bizarre statement of the night had to have been Mitt Romney's historically-ignorant and righteously-indignant avowal: "I don't think we've seen in the history of this country the kind of attack on religious conscience, religious freedom, religious tolerance that we've seen under Barack Obama."

To borrow a schtick from Saturday Night Live in response: "Really, Mitt? Really?!? Are you really that ignorant of, say... just for example... the history of the Mormon Church and the federal government? Google 'Mormon Wars' sometime, you may learn something. Really! Why was your father born in Mexico, again? Oh, that's right, because his parents were there because of what they saw as religious persecution by the United States government. Really. The federal government didn't just specify what health insurance a Mormon hospital or charity offered their employees, the federal government dictated to the Church of the Latter-Day Saints what religious tenets they could believe in, and which were unacceptable to federal law. Remember that? You might want to check your church's history, there, Mitt. Or if that's too tough for you, how about the history of your own family?!? I mean, really."

Mitt's not only provoking the likes of me to rebut him, he also provoked an average citizen named Chuck Smith to wager his own $10,000 bet with Romney over a complete falsehood Mitt's been using out on the campaign trail -- how Barack Obama "promised" everyone that unemployment wouldn't go over 8 percent if his stimulus plan was passed. While quite obviously an amateur attempt (it even points out its own editing mistakes within the video itself), this video is well worth watching in its entirety. Shows what an average Joe can do, when the media completely falls down on its job (as usual). It even includes footnotes and references, to prove exactly what the guy is claiming. Even if he doesn't win his bet, the video itself is priceless.

In other weird news, the Smithsonian held a groundbreaking ceremony for an African-American museum on the National Mall, which our first African-American president attended with his wife. When they called for people to come up and turn some ceremonial dirt with shovels, Barack Obama stood up and was then told that he wasn't invited to participate -- even though Laura Bush was. Weird, eh? The Smithsonian flunky who told the president this could have just as easily turned it into a memorable photo op -- by allowing Barack Obama and Laura Bush share the same shovel -- but instead told the president to sit back down.

Meanwhile, a Republican representative had to apologize for threatening to kill some Democratic senators, but (sadly) these days, that's not as weird as it should be.

Even the conservative press (weirdly enough) reported that pixie dust and magic simply doesn't work when it comes to budgeting, and that under the Republican presidential candidates' budget plans the national debt would explode at a rate much higher than President Obama's budget. But wait... I thought tax cuts paid for themselves? Weird, eh?

And finally, in one of those stories that seems inevitable (but only after it appears), an artistic couple has created a portrait of Rick Santorum made up entirely of tiny images of gay porn. Salon has the story (which can safely be read at work, if you don't click to expand the image to giant-sized... just in case you were wondering).

Enough weirdness, though, we've got to move along. I feel a gigantic rant coming on, so we'll try to keep the award section brief this week.

Most Impressive Democrat of the Week

Following the precedent we set last week, we award all the Democrats in the Maryland state government who just passed a gay marriage bill a Most Impressive Democrat of the Week award. Because the "Golden Backbone" statuettes are not cheap, we will be sending only one of them to Governor Martin O'Malley, who is expected to sign the bill soon (if he has not already done so). Last week, Washington state did the same thing, but many "journalists" didn't notice, apparently due to their not being able to find the West Coast on an American map. Maryland, however, is right there in the middle of the East Coast, so this one simply couldn't be ignored. Well done, Free State Democrats!

We have a second Most Impressive Democrat of the Week to hand out as well, to the witness who was denied a chance to be heard at the Republican's male-dominated hearing on women's health issues. House Democrats held their own "hearing" just to give a chance for Sandra Fluke to tell her story. Minority Leader Nancy Pelosi deserves an Honorable Mention for staging this event, but the MIDOTW award goes to Fluke herself, for telling a very compelling and important side of the story which the Republicans really don't want the media to pick up on: Birth control pills are not just used for birth control. We're just assuming Sandra Fluke is a Democrat, but this is probably a pretty safe assumption, we feel.

Friday Talking Points

Volume 199 (2/24/12)

I'd like to begin today's talking points with an admission that maybe I was wrong.

The "maybe" in that sentence stems from the future's unpredictability. The jury won't be completely in on the issue until the election results (and exit polls -- from such states as Ohio and Pennsylvania, in particular) are released this November. So it's still too early to say.

Two weeks ago, in this space, I wrote a column entitled "White House Fumbles." The political issue I felt they fumbled was their announcement of the new health insurance regulations which stipulated that women's preventative medicine be covered by all non-church employers.

I felt that they fumbled because they were seemingly caught so flatfooted at their response to the controversy, which should have been at least somewhat predictable (to say the least). My column was written on the same day the White House announced their compromise, which still stood strong for women's health care.

Since then, the issue has grown and morphed -- to the Democrats' advantage, for the most part. In other words, it seems to not only be working out OK politically, but actually energizing the Democratic base over the issue. All of a sudden, women's health is in the news, and the Republicans are not looking good.

Which is why I'm starting with my "maybe I was wrong" statement. Maybe this is going to wind up being a good thing, even if (as I still believe) it could have been handled better at the beginning. But that's water under the bridge now. Instead of looking back, let's look forward instead.

If this is going to be a good issue for Democrats to make some political hay on, then I offer up the following as how I would go about doing it.

Democrats, so far, seem to be winning the framing battle on this issue. This is astonishing enough, since they're usually the ones playing catchup to the Republicans. But Democrats need to take it up a big notch, here. Instead of fighting each individual battle, Democrats need to offer up a big picture -- a very ugly big picture -- of what the Republicans' overall goal truly is. Thankfully, these dots are not that hard to connect right now. So many stories have gone Democrats' way in the past few weeks, that the time is ripe to hammer the big picture home in a way that will have Republicans playing defense from now until November. I offer up the following rant as an example of how to do so effectively.

Is this hyperbole? Perhaps. Is it demagoguery? You betcha. But just imagine if every Democrat starting using one simple phrase over and over again. Imagine the media picking up on this phrase. Imagine the media then declaring this election the "Year of the Woman Voter." Imagine, if you will, a strong-willed female Democrat saying something along the following lines in the near future.

My "War on Women" Rant

The Republican Party is currently waging a war against women. They want to take America back a half-century or more, and Democrats are not going to allow them to do so!

When I see a congressional hearing called on women's heath with a panel of five old men, I wonder what the Republicans are thinking. When the state of Virginia contemplates passing a law which would force doctors to invade a woman's body for no medical reason at all, I think Republicans have forgotten all those talking points about not allowing government to get between a doctor and her patient -- talking points they used to love to utter. When I hear the likes of Rick Santorum weighing in on what tests a pregnant woman should have, I almost expect him to say women should stay eternally barefoot and pregnant. That's how bad it has gotten.

Republicans are waging a war against women. Furthermore, they are attempting to wage a war against sex. Why should some politician be able to tell any American woman how to run her life? Why should birth control even be an issue in politics in the 21rst century? Why should Republicans -- who are supposed to be for getting government out of people's lives -- want Big Brother in every doctor's office?

When Republicans held their "no women allowed" hearing, the witness Democrats wanted to question was excluded. Democrats held a separate hearing for her this week anyway, and she testified about a friend of hers who could not afford birth control which had been prescribed to treat ovarian cysts -- and about the sad outcome. Contraceptive pills are prescribed on a daily basis for medical issues which have nothing to do with sex and which save women immeasurable suffering. But now Republicans are pushing to allow any employer in America to prevent a female employee's health insurance from covering such life-saving medicine. This is a war on science. It is a war on women.

Republicans say employers should be allowed to follow their "conscience" in such decisions. Where will this end? Will employers be able to limit AIDS coverage, because they do not morally approve of people who get AIDS? What about insurance coverage for sexually transmitted diseases? Should employers be allowed not to provide such coverage? What about an employer who doesn't believe in any medical intervention except prayer? Should he be allowed to limit his employees to "prayer-only" health insurance?

There's a reason why we have public health laws -- to protect the public's health with the best science available. All of the public. Even women! If this science tells us that an ultrasound is not necessary for a medical procedure, then why should politicians be able to override the doctor? Most states have laws against practicing medicine without a license, but Virginia lawmakers feel free to dictate medical treatments from their statehouse. Why aren't these lawmakers all being arrested for impersonating doctors?

It's a war against women, plain and simple. When the national public learned that the procedure mandated by the Virginia law would have forced women -- against their will -- to have a transvaginal ultrasound that was completely medically unnecessary, America recoiled in revulsion. So the governor backed off, and will now only force women to have an external ultrasound in such a situation -- a procedure which is not only completely unnecessary, but actually totally useless to spot an embryo which is too small to be seen by such methods. It is a war against medical science, and common sense.

The Virginia governor is attempting to put this genie back in the bottle. His statement read, in part: "No person should be directed to undergo an invasive procedure by the state, without their consent, as a precondition to another medical procedure." But why would the state mandate any medical procedure -- without the patient's consent -- as a precondition to another medical procedure? What has happened to the conservatives? This is your idea of government getting out of people's lives? This is supposed to be conservatism? Barry Goldwater must be turning over in his grave, by now.

The state of Virginia backed off on forcing doctors to perform what Virginia law describes as "rape" on an unwilling woman. I suppose we should be thankful that now the new law will only shred the Fourth Amendment to the United States Constitution instead. Why should the government force a doctor to perform a search of a woman's body for her to get a legal medical procedure done? It boggles the mind.

As if all this weren't enough, it seems now that the Republican Party has opened up a new front of the war on women: a war on, of all things, the Girl Scouts. This is in addition to their war on Planned Parenthood -- often the only opportunity women have for their own health care for miles around.

This has gone too far. Republican men who sit in legislatures should wake up and realize that a woman's health -- including her reproductive health -- is her business and hers alone to decide. Any woman should be able to go to her doctor and have whatever medical care the two of them agree upon without moralizing governmental interference. This is about freedom, plain and simple.

I have a message for the Republicans who think this is some sort of political football which will help their party out -- you know what? Your wives, your sisters, and your daughters are having sex. Deal with it. It is a part of human nature. This is America, and they are free to do so, and you have no right to interfere with this decision in the slightest.

If your daughter is violently raped, then Democrats believe she should be able to get an abortion, so she does not have to spend the rest of her life with the consequences of this violent crime against her body. Deal with it. If American women want to use birth control, guess what? It is entirely her decision to do so. If a doctor prescribes a birth control pill to a woman because if she does not take it she may otherwise suffer, then -- hold onto your hats -- her employer doesn't get to be in the same room with a veto over her freedom. If a woman wants a procedure which is legal, then you don't get to attempt to shame her into giving up this freedom -- and especially not by trouncing all over doctors' First Amendment rights to say what they wish to their patients.

While a few high-profile stories have been in the media in the past few weeks, what has been largely ignored for the past year or so is the continuing Republican assault on women's liberty and women's freedom. The Virginia law horrified many, but the exact same law is already in place in other states. Because men decided they knew best. Republican men, for the most part.

On top of this, women are forced to go through "waiting periods" in many states before getting a legal procedure done -- for no medical reason whatsoever. If you have to travel hundreds of miles to get an abortion, this is supposed to make the price of doing so too high for some women. No other medical procedure in America is treated this way -- only the one Republican men don't approve of.

These men are guilty of practicing medicine without a license. They are guilty of attempting to condemn women to death for lack of medical care -- as would surely happen if abortion were made illegal. They are guilty of attempting to legislate coercive rape upon women patients. They are guilty of attacking women's rights and women's health care -- every chance they get.

This is a war on women. We're fighting on the side of women. Maybe we should fight back on their battlefield. Maybe we should start passing laws requiring an anal probe for anyone who gets a Viagra prescription. Maybe we should hold a hearing on prostrate health and invite a panel of all women to testify why they're morally opposed to treating it. Maybe we should make testing for sexual diseases mandatory for anyone running for office. Maybe we should have a two-day waiting period for anyone who wishes to enter a strip club. You see how ridiculous it sounds when the shoe's on the other foot?

Democrats are on the side of women. We're on the side of science. We're on the side of doctors and patients being free of having Big Brother or their employer in the examination room. We're on the side of women's health -- all her health, and not just the pieces of it which old men decide are proper. Men like Rick Santorum, who is now apparently against pre-natal testing -- which even Sarah Palin herself was thankful for when she was pregnant. Republicans want to turn back the clock on women's health a half-century or more, and we will not let them!

I invite all of America's women voters to ask yourselves: Should I vote for the party which stands firm for my health care, or should I vote for the party which wants to deny me the freedom and liberty of making up my own mind what is right for me? Should I vote for those who are on the side of science and full health care rights for all, or should I vote for those who want to deny me access? Should I vote for those who want to force my doctor to penetrate me against my will, or should I vote for those who are outraged by such a concept?

The choice should be obvious. If you're for continued sexual freedom, and continued liberty in your own health care, then I strongly urge you to vote for those who are not actively trying to legislate these things out of existence -- in a word, Democrats. You are the foot soldiers in this war against women, and if you speak loudly enough, even Republican politicians will notice you in November.

Property Management Challenges Investors - Real Estate Economy ...

Only two out of ten of the nation?s 8.7 million single family rental properties are managed by professionals; individual owner/ investors take care of most of the rest.? Last year only about a third of single family rentals, 35 percent, made a profit on their rental income.? Some 2.3 million lost money for their owners.

Is there a relationship?

As more and more real estate investors abandon flipping for a buy and hold strategy, they are discovering that becoming your own landlord may end costing them much more than management fees. Some 18 percent of owners spend 25 percent or more of their time managing their rental property and 20 percent spend 20 percent or more of their rental income on maintenance, according to a major study last fall by the Census Bureau.

Many investors have the time and expertise to manage their properties, but millions, especially newcomers, assume land lording will be easy, only to learn otherwise.? One out of four single family tenants stays less than a year. ?One in four is delinquent on their rent at least once every two years.? Twelve percent of individual owners must visit their property once or more a week.

?When I bought my first property and placed my first tenant?I was a complete disaster! No point in beating around the bush right! As a real estate investor, I learned just enough to be dangerous and I certainly was,? wrote investor Chris Clothier of Memphis Invest on the Bigger Pockets blog.

?Things may go along fine for a while and then they discover asbestos in the basement or they have to evict someone and have no idea about local laws and regulations.? Or they give the tenant permission to paint a room and he kicks over a can of paint.? Who?s going to pay for new carpeting?? Not the tenant, who can?t afford it,? said Kirk McGary, CEO of Real Property Management, the nation?s largest property management company and one of the few that has embraced the single family market.

Single family rentals have transformed McGary?s business, which now boasts 200-plus franchises and is growing quickly.? Yet he admits the single family business is not easy.? ?In one apartment building you might have 500 tenants, one owner and one location.? With single family, you have 500 owners and 500 locations for the same number of tenants.? McGary?s franchises get more than 10.000 calls from tenants a day.

Unlike Real Property Management, most traditional property management companies, which are structured to serve the multifamily market, aren?t interested.? The demand is being met largely by small, local businesses and even some real estate brokerages, notably several RE/MAX and Coldwell Banker franchises, are getting into the management business to serve their investor clients.? However, these are found primarily in the foreclosure hotspots? California, Phoenix, Las Vegas, Florida, Illinois, Ohio, Michigan.? Elsewhere, investor/owners have more limited choices.

The real estate investment boom has also created a growing business for rental web sites as well.? At least one national website, Chicago-based RentalHomesPlus, has dedicated itself to serving the single family rental market. Jon Pastor, CEO of Rent Jungle, which lists over 700,000 rentals from 12,371 web sites, confirms that the number of single family rentals is very large and growing? up 15 percent in the last 18 months.

With 6.8 million single family rentals managed by owners and the number growing every day, the property management problem is creating challenges for investors and opportunities for entrepreneurs.

Samsung demos new 32nm quad-core Exynos ahead of MWC

If you were lucky enough to be at the International Solid-State Circuits Conference, then you might have caught a glimpse of Samsung's latest sliver of mobile silicon. The as yet unnamed Exynos parts will come in dual- and quad-core configurations running at up to 1.5GHz. Perhaps the most important change though, is the switch from a 45nm manufacturing process to 32nm. That means smaller parts that draw less power, while delivering better performance. The A9 cores are paired with Sammy's own GPU, an OpenGL ES 2.0-capable chip with four pixel processors. According to the company, the new CPUs deliver a 26 percent boost in performance and up to a 50 percent increase in battery life. Of course, we'll have to wait to run our own benchmarks to confirm that lofty claim. Who knows, maybe we'll see it show up in some phones at MWC.

(AP) ? Wal-Mart Stores Inc.'s business offers valuable insight into how Americans are reacting in the slow economic recovery: they will spend, but only if they believe they're getting the lowest prices.

The world's largest retailer guaranteed the lowest prices on merchandise this past holiday season. As a result, bargain-hungry shoppers flocked to Wal-Mart in the fourth quarter, helping it to record its first increase in store traffic in at least two years. But the company's margins suffered.

"Core customers remain cautious about their finances," said Mike Duke, Wal-Mart's president and chief executive, in a statement.

The new consumer behavior is likely to have an impact on companies of all shapes and sizes as they struggle with how to lure shoppers in with low prices without cutting them so much that it erodes profits. Wal-Mart, which draws nearly 10 percent of all nonautomotive spending in the U.S., highlights the compromise companies have to make when they focus on rock-bottom pricing in the still-weak economy.

Wal-Mart has struggled since the recession as its core low-income customers at its U.S. namesake business have been hard hit by joblessness and other challenges. Meanwhile, the business, which accounts for 62 percent of the company's revenue, had veered away from its low-price strategy.

In the third quarter of last year, Wal-Mart reversed a string of quarterly sales declines at stores open at least a year ? an indicator of a retailer's health. Wal-Mart ended the slump that lasted for more than two years in part by refocusing on offering low prices throughout the store, instead of temporarily slashing prices on select items.

In the fourth quarter, Wal-Mart continued the trend. It posted its second consecutive quarterly gain in revenue at stores opened at least a year at its namesake Wal-Mart stores in the U.S.

Wal-Mart said Tuesday that revenue at stores open at least a year at its U.S. namesake business rose 1.5 percent in the quarter, slightly below the 1.6 percent gain analysts polled by FactSet had expected. Overall, it's U.S. business had a 2.1 percent increase in revenue at stores opened at least a year, including a 5.4 percent rise at Sam's Clubs.

Net sales, excluding membership fees from its Sam's Club division, rose 5.9 percent to $122.28 billion. Analysts had been expecting revenue of $123.9 billion.

While sales rose, margins fell. Overall gross margin was 24.3 percent in the quarter, down from 23.9 percent in the year-ago quarter.

The discounter, based in Bentonville, Ark., said net income was $5.16 billion, or $1.50 per share in the three months ended Jan. 31. That compares with $6.05 billion, or $1.70 per share, in the year ago period.

Excluding certain tax matters and real estate transactions, Wal-Mart recorded $1.44 per share in the latest quarter. The year-ago results included benefits from discontinued operations; excluding those benefits, earning were $1.41 per share. Analysts were expecting earnings per share of $1.46.

"They're working extremely hard just to see improving sales," said Brian Sozzi, chief equities analyst at NBG, an independent research firm. "But it's coming at the expense of profits on each sale. Just imagine if they weren't doing this."

Reclaiming its reputation as the lowest-price leader is critical for Wal-Mart to sustain the upward sales trend. So, going forward, Wal-Mart officials say the company will keep looking for ways to cut prices.

"You can expect us to invest even more in lower prices," Duke, Wal-Mart's CEO, said.

For the current quarter, Wal-Mart expects revenue at stores opened at least a year to be flat to up 2 percent. For the year, it expects earnings per share to be $4.72 per share to $4.92 per share. Analysts had expected $4.90 per share.

During a media call with reporters Tuesday, Charles Holley, chief financial officer, said that January has the best performance in the quarter, reflecting the sales momentum the discounter is enjoying.

"I do think there's a new normal with customers," Holley he said during a call with reporters Tuesday. "The markets are more volatile. Gas prices are more volatile. Customers are looking for new ways to save money because they don't know around the corner."

'Yes, we need retirement villages'

Few housing options for the old in S'pore, says senior-living consultant

LAST Friday's Budget bestowed a hamper full of goodies on older Singaporeans.

Families caring for the frail elderly are in for a windfall, with more state subsidies for care in community hospitals, nursing homes and even at home. For the first time, even the richest can get state subsidies for stays in community hospitals.

In this 'season of plenty', it may seem politically incorrect to talk about some seniors who feel short-changed about retirement options.

But international senior-living consultant Tan Kee Hian, 57, believes this is as good a time as any to discuss the relative shortage of living choices for Singapore's growing group of more affluent, older folk.

He says many seniors in the top fifth to 20th percentile of the income scale would gladly part with their savings to live in retirement villages (RVs) which offer professional care and support services such as an on-site round-the-clock emergency response system.

While such communities are common in most developed countries, there are none in space-starved Singapore. Both the public and policymakers seem to prefer 'ageing in place' - where people live out their last years in the community rather than in institutions.

But Mr Tan says that while boosting community resources to enable the majority to age in place is a 'basic necessity' here, no one solution can fit the needs of all seniors.

'Ageing in place assumes that everyone has similar needs - and that's a very big assumption. Given the diversity of our First World society, we deserve more choices.'

The veteran management consultant is convinced there is a need and even a demand for Western-style retirement villages, and hopes to develop Singapore's first such project.

He says those who argue that there is absolutely no demand for retirement villages here are divorced from reality.

'If you repeat a myth often enough, it gets mistaken for a fact.'

Over the past two years, he has rigorously researched and written a proposal for a $70 million to $80 million retirement living project in Jalan Jurong Kechil, a site the Government has earmarked for the purpose.

All he needs is an investor who is willing to put in up to $25 million. The rest will be made up by bank loans and he is willing to put his cost calculations up for thorough scrutiny.

The Harvard Business School-trained expert says he has done his homework and pulls out a consultant's most handy tool: statistics.


Significantly, that is far higher than the 12 per cent of older folk who actually live in such communities in countries like the United States.

There are nearly a million baby boomers here, born between 1947 and 1964. The first cohort turns 65 this year, which Mr Tan sees as an 'inflection point' in this nation's history.

Singapore's population of those aged 65 and above is set to double to 600,000 over the next decade and triple to nearly one million by 2030 - that's a faster rate of ageing than in virtually every other nation in the world.

He says some demographic micro-trends have reinforced the need for RVs.

The number of older folk here who live alone or with their elderly spouse doubled to nearly 100,000 between 2000 and 2010.
The number of those who have mobility problems and are 'semi-ambulant' has also doubled to around 36,000.
The exploding numbers of these two groups - which have traditionally formed the core client base of retirement communities overseas - have far outstripped the overall growth in the senior population, which increased by around 50 per cent to 344,000 over the past decade.

The exploding numbers of semi-ambulant seniors are a particular concern for Mr Tan, who is a baby boomer himself.

Although they are in relatively good health, they may end up living in expensive nursing homes simply because they are immobile.

'Most such patients overseas would be living in retirement communities, saving their health-care institutions taxpayer dollars.'

Mr Tan, who returned to Asia in 2006 to be near his ageing parents after 35 years in Europe and the US, points out that he is often asked why he wants to 'break up families and hurt inter-generational ties' by advocating RVs.

'Just look at the statistics,' he says. 'The number of older folks who want to live by themselves may well explode. And as things stand now, many may not be getting the services they need.'

Indeed, growing groups of seniors either do not have children or do not want to depend on them as they age. Many - like Mr Tan himself - have children who live and work overseas.

Small wonder then that, according to the baby boomers survey, nearly 75 per cent of all respondents - and 85 per cent of university graduates - said they wanted to live on their own during their golden years.

About a third of those aged 65 and above already do so now.

But even if the demand is there, will older people have enough savings to pay for such resources?

Mr Tan reckons they will. Singapore, after all, has the highest proportion of millionaires in the world, with one in six households on that rich list. The number of those who earn above $10,000 a month has increased by nearly three-fold to 140,000 in a decade.

According to the MCYS survey, 12 per cent of baby boomers - or potentially 120,000 people - have gross monthly incomes of $7,500 and above.

Interestingly, the father of two acknowledges that it is not the Government but the private investors who should test the market by setting up Singapore's first retirement village.

To be fair, the Government has already taken the initiative to inject some variety in elder-living options, particularly those that cater to middle- to lower-income Singaporeans.

On Sunday, it launched Singapore's first group home for low-income elderly, where small groups of older folk who have little or no family support can share rental flats.

The Housing Board (HDB) has ramped up the supply of studio apartments for the elderly - and latest take-up rates are above 90 per cent.

Meanwhile, MCYS recently piloted 'seniors service centres' for residents in these apartments, providing information and referral services, organising social services and responding to emergencies.

But what these facilities do not cater to yet is more 'sophisticated needs', says the professional, who would love to live in such a facility one day.

For instance, there are no provisions for on-site nursing care.

There are older folk, for example, who may want care facilities within their condominiums - rather than in the neighbourhood. Some might not want to - or be able to - walk even 400m to the nearest polyclinic.

'A retirement village combines elder-friendly housing with care and community engagement services, together with professional management,' says Mr Tan. 'And no facility in Singapore currently provides all three.'

While senior activity centres might provide referral services, they certainly don't have a 'village manager' whose primary duty is to ensure the well-being of seniors living in his village.

So given that there seems to be a market demand, why has no developer bitten the bullet so far?

Mr Tan believes there is a 'fundamental mismatch' between what property developers traditionally do and what seniors want.

Developers want to sell houses, but seniors want to buy care; developers want to build, sell and move on; seniors want a provider who will stay and ensure a specified quality of care and services for the rest of their years - for many, many years.

'Developers love more of the same, but a retirement community is really a new market space,' says Mr Tan. 'We need a new breed of champion investors - and I hope we find them fast.'

Senior-living specialist
MR TAN Kee Hian, 57, is a senior-living consultant who has worked on retirement and community living projects in Singapore, Malaysia and China.
In Singapore, he has been adviser to a proposed state- sponsored communal living project, to be run by a voluntary welfare organisation. He has drawn up detailed plans for what could be the island's first retirement village and is looking for private investors.
Mr Tan is also on the advisory committee of the non-profit group RSVP Singapore (The Organisation of Senior Volunteers).
Before moving to Singapore in 2006, he spent three decades as a management consultant working out of London and Boston in senior positions in companies like Gemini Consulting (Cap Gemini), A.T. Kearney and Shell International Group. He was the global leader for Gemini's health-care and life sciences practice. Since 1996, he has headed his own consulting firm, KHT and Co, which has had 40 clients in 15 countries.
The Malaysia-born Singapore permanent resident was an Asean scholar at National Junior College and went on to study engineering at Imperial College, London. Later, he received an MBA from Harvard Business School, where he was in the top 5 per cent of his cohort.
Mr Tan is married to Madam Chia Swee Tuen, 55, a Singaporean piano teacher. They have two grown-up children.

Q&A: What baby boomers want

People have been talking about retirement communities here for 20 years. So why has nothing materialised yet?

A: Twenty years ago, the retirement community idea was clearly ahead of its time. The generation that grew old then had very different cultural values, needs and preferences. Older folk were less-educated, less-travelled, had limited means, and were quite happy sitting at home, chatting occasionally with or tending to grandchildren and family members.

Today's and future generations of seniors - the baby boomers - are very different. They are better-educated, are used to more sophisticated lifestyles, and have worked hard to squirrel away nest eggs. They have seen the kind of services seniors enjoy overseas and want such professional care services in Singapore. They don't want to depend on their children. They have hobbies, they want social engagement. Just interacting with - or depending on - family is not enough for them. So, yes, their needs and expectations have changed - and become more Western, if you will.

What would you say to potential investors who argue that there is no money to be made in retirement villages (RV)?

A: Whether you make money or not will depend on what you offer. Like any value-added product or service, people are willing to pay to get what they need and want. A cursory look at the success of HDB's studio apartments will confirm that customers will pay a premium for the right solution in the senior-living market in Singapore.

I believe that unlike younger home buyers, baby-boomer seniors who buy a unit in a retirement village are not as interested in the size of their home or in its investment value. They want the convenience of having services on-site - be it a shared nurse or a domestic helper or a manager who ensures your day-to-day well-being, a wide range of social activities and a vibrant community of like-minded people.

At the same time, there are frills that seniors don't want or care for, such as marble tiles, grand water features or large, expensively fitted kitchens.

According to my analysis and calculation, with the right business model based on a deep understanding of the baby-boomer seniors, an RV can deliver up to 20 per cent more profit than conventional condos in the Upper Bukit Timah area. Of course, this is not a one-off project, as success will provide a strong credibility to expand and gain a disproportionate share of the potential market, which I estimate to be well in excess of 10,000 by 2030.

Are retirement villages necessary in an urban setting where access to community medical and social services is just a stone's throw away?

A: To answer this, we must go back to how seniors define their needs and preferences. Experiences from more advanced countries show a strong preference for on-site provision of care services and convenience, even in urban settings such as London and New York.

For the record, being land-starved has nothing to do with the viability of a retirement village. Everyone has to live somewhere, whether it's in a traditional home like an HDB flat or condominium or an RV apartment. Who says RVs have to be sprawling complexes spread over 200 acres (80ha) like in Australia or rural USA?

Vertical retirement villages - which are basically elder-friendly condominiums with comprehensive on-site services - are also a viable option and there are many such complexes in both Europe and the US.
